Diagnosis: Increased prevalence of Type 2 diabetes among African Americans aged 65 and older related to higher rates of obesity and unhealthy habits, decreased access to primary, secondary, and tertiary healthcare resources, and increased physiological stress influenced by societal and genetic risk factors as evidenced by statistical data indicating higher morbidity and mortality rates as well as lower health outcomes compared to other community members.

Focuses on interventions and evaluation. Based on your community diagnosis you will identify, define, and describe in detail from the Public Health Nursing Wheel of two Interventions at a community level.

Be sure to describe the role of the nurse in these interventions. One should address the policy level. Interventions should be specific and detailed. For example, if screening is an intervention describe the tool that would be used, the setting, the process and the follow-up (referrals).
