Introduction – This should show that you have fully understood the assignment brief and that you are going to cover everything required.
Introduce your clinical area and provide some context / background.

• Describe the change you want to implement. Provide the evidence base for the change.
• How does this change improve patient care / the patient experience?
• Identify some of the enablers and barriers to making this change (you may find it useful to undertake a force-field analysis / SWOT analysis – this can be presented as an appendix).
The introduction should therefore explain the significance of undertaking the work reported on, not only in terms of informing decision making for practice development but also with regard to recognising your own continuing professional development needs. In other words, what are the relevant themes and issues to be explored in the report in relation to implementing a change in practice and why are you being asked to investigate these now? You must then clearly identify the practice change or innovation you have chosen to focus on before outlining the basic structure of the report. (200 words)

Main body 1 – Context / Background – Propose an evidence-based change you would like to see implemented in your clinical / service area – Many reports will provide some background information within the introduction but if a more detailed review of previous research and supporting literature is required, this should be placed in a separate section.

Change management
• With reference to theories and models of change management:-
• What change model would you use and why?
• What advantages has your chosen model over other potential models?
• What are the limitations of your chosen model?
• How will you implement the different stages of the model – be specific, link . Unlike an essay, headings and sub-headings should be used to clearly indicate the different sections. Your discussion should demonstrate your understanding of key factors in practice likely to influence the implementation of your proposed change a critical approach in which appropriate change management theories are drawn upon to support your perspectives. Your sub-headings should be used to indicate the factors you feel are of specific concern to the implementation of your proposed change given the nature of the change and the clinical area targeted. Main body 3 – Leadership – Critically analyse the leadership style you would use in conjunction with the change model / methodology – This is another important part of your report and is where the issues outlined in the introduction are expanded upon.
Leadership
• You should consider the theoretical differences between leadership and management. Are differences important – if so why?
• You should reflect on the different leadership styles in the literature and identify the style you think would be most effective in engaging the team in taking forward this particular change.
• Give reasons for your choice, why will this style be more effective in this instance?
• Consider some of the challenges for leadership arising from factors such as budget restraints / staffing levels and skill mix / individual staff resistance as well as resistance as a result of embedded culture.
.Unlike an essay, headings and sub-headings should be used to clearly indicate the different sections. Your discussion should demonstrate your understanding on the role of leadership and the impact of different leadership and management styles in addressing your proposed change. Again, it is not sufficient here to simply describe a situation but rather you must evidence a critical approach in which appropriate leadership and change management theories are drawn upon to support your perspectives. Although leadership will be the key theme to run throughout this section, your sub-headings should be used to indicate the factors you feel are of specific application to the implementation of your proposed change given the nature of the leadership style and the clinical area targeted. Identifying and concentrating only on how leadership influences change will ensure your discussion is sufficiently detailed and analytical to indicate depth of understanding. (850-1000 words)

Conclusion and evaluation – The conclusion is used to summarise the findings and implications of the report and reinforce for the reader the value of the work undertaken. Within your conclusion you should therefore make explicit the links between your discussion and the overarching objectives of conducting this investigation as you make the transition to professional practice. Also, how you would evaluate the effectiveness of your change. Other factors you may wish to consider include the relevance of issues such as organisational culture, the environment of care, communication, resource allocation, team and multi-professional working practices. The conclusion should not contain any new ideas or information that has not been mentioned in the main body of text. (500 words)

References (not in word limit) – Follow Harvard referencing guidelines.

Appendices (not in word limit) – The content of any appendices should be referred to at appropriate points during the course of your report but only use this strategy if you are certain the documents or data you include add to the reader’s understanding of the report .
