Practicum class #1. 1. What are your initial thoughts after reviewing the AHRQ website?

I
believe health literacy is a topic that needs to be addressed so
patients can have a basic understanding of their health disparities,
prescribed medications, and required preventative measures to allow for
optimum health and well-being. Primary care requires providing care to
diverse patients such as, adults, pediatric, geriatric, and often
patients that do not speak English as their primary language. Access to
healthcare and comprehension of health information can be confusing and
often misconstrued. The Agency for Healthcare Research and Quality
(AHRQ, 2020), has designed tools to bridge the gap between providers and
their patients to enhance health literacy. These precautions are
designed to simplify communication, provide easy navigation in the
healthcare environment, and provide support for patients desiring
improvement in their health (AHRQ, 2019).

2. What are some strategies you can incorporate in everyday practice to assist with health literacy?

I
am of fan of how the website discuses simplifying communication. I live
in Mississippi in a very rural area. One of our yearly goals where I
currently work is to have 75% of our patients enroll in My Chart, which
is our online charting database. Internet accessibility is an issue
where we live. This is an unattainable goal for where we live. Practical
and simplified communication is what I hope to promote in my future
practice. Why make things more complicated for our patients than they
have to be. For example, initially COVID vaccinations in our area were
only available if you scheduled your appointment online. There were
several older church members that were unable to be vaccinated until
younger members of our congregation stepped in to help. My favorite
information I read on this website was to assume that everyone might
have difficulty understanding and creating an environment where all
patients can thrive (AHRQ, 2020). Provide simple written instructions
when necessary; use graphics where possible (American Mobile, 2021). An
easy to follow list of the basic concepts you have discussed will help
to ensure compliance with their plan of care. Also, pay attention to
facial expressions so anxiety, confusion, and thoughts of worry can be
addressed. Repeat essential information and allow the patient an
opportunity to ask questions. Utilizing the teach back method can
promote comprehension and may reveal topics that were not understood
(Alper & Institute of Medicine, 2015). If there is any doubt that
what you have said is not understood, it is imperative to go back over
the information until clarity is reached.

Practicum class #2. . What are your initial thoughts after reviewing the AHRQ website?

Health literacy is broken down into two categories, according to the
CDC (2021), personal health literacy and organizational health literacy.
Personal health literacy is the ability of a patient to understand and
comprehend the given information by a healthcare professional to
determine best course of action to treat their various health problems.
Organizational health literacy is degree to which healthcare
organizations present and organize the information given to patients.
AHRQ Website is an exceptional tool for healthcare professionals to
learn ways to incorporate simplicity into a complex medical system. I
think as healthcare professionals, it is our job to explain medical
practices in the best way possible that a patient can have all the
information needed to make a solid informed decision on their health
needs. As nurses, we are taught to teach and educate. Our greatest
impact on the healthcare system is our ability to teach and educate
patients on their disease processes and the treatments that are
available to them. I think nurses are the greatest prepared healthcare
providers to extend health literacy to the masses.

2. What are some strategies you can incorporate in everyday practice to assist with health literacy?


The goals of incorporating health literacy into every day practice
start with developing information that is accurate, easily accessible to
patients and provides an action plan to prevent diseases. The
information should provide the patient with the diagnosis, what are the
treatments for this diagnosis and explanation of the risks to their
health. The information provided to patients should come from reputable
sources who are actively researching and providing up to date
information to the public about various diseases, such as the CDC and
other public health organizations. The information provided should be
provided on a 5th grade reading and comprehension level. The plan should
be approved and delivered by all members of the health care team.
Patients must also be provided with services that explain the various
materials and can coach the patient, such as a diabetes educator or
dietician. Referrals should be made appropriately by healthcare
professionals when there is a knowledge deficit that is identified.
